If your physician has joined Complete Concierge Care and you would like to continue to be their patient or if you have any questions about enrollment, please also fill out the Patient Enrollment Form. * Current or prior patients of Dr Ana Paunovic and Dr. Allison Hockman can skip the meet and greet visit with their physician and only need to complete the enrollment paperwork to enroll. Once the enrollment paperwork and membership payment are received, we will confirm the enrollment and you can schedule your first visit.
