Lifestyle Medicine & Longevity Care:
A Proactive Model
Lifestyle Medicine & Longevity Care is a specialized offering from Dr. Brad Moore for individuals seeking a proactive, science-driven approach to optimizing health, vitality, and long-term quality of life. Dr. Moore integrates evidence-based Lifestyle Medicine with personalized medical care to address the root causes of chronic disease and improve health span. The focus extends beyond disease prevention to improving function, resilience, and overall health span at every stage of life.
Many of the conditions that compromise long-term health—including cardiovascular disease, metabolic dysfunction, diabetes, obesity, and hypertension—are strongly influenced by lifestyle factors. This care is grounded in the understanding that nutrition, physical activity, sleep, stress management, and social connection are powerful medical tools when applied intentionally and consistently within a clinical framework.
Dr. Moore partners closely with patients to develop individualized care plans that align medical treatment with realistic, sustainable lifestyle strategies. This approach reflects the core principle of Lifestyle Medicine: that how we eat, move, sleep, manage stress, and connect with others plays a central role in preventing, treating, and, in some cases, reversing chronic disease. Care is personalized, evidence-based, and built on a collaborative physician–patient relationship.

— LIFESTYLE & LONGEVITY CARE
Philosophy of Care
This practice is built on the belief that lifestyle is the most powerful—and most underutilized—tool in modern medicine. Nutrition, physical activity, sleep, stress management, social connection, and daily habits profoundly influence the prevention, treatment, and in some cases reversal of chronic disease.
We prioritize addressing root causes rather than simply managing symptoms. Medications and procedures have an important role in care, but whenever possible, we focus first on optimizing lifestyle to improve health, function, longevity, and quality of life.
Our role is to partner with you in all three areas—providing evidence-based guidance, helping clarify goals and motivation, and translating recommendations into practical, achievable steps that fit your life.
This is a collaborative, high-touch model of care that values time, trust, and continuity. It is best suited for individuals who want to be active participants in their health and are willing to engage in guided lifestyle change.
We believe that meaningful, lasting behavior change does not happen through information alone. Sustainable change requires three essential elements:
— LIFESTYLE & LONGEVITY CARE SERVICES
Our Services
We offer two distinct but complementary service lines, each grounded in the same evidence-based lifestyle medicine principles.
— CONCIERGE MODEL
Lifestyle-first Primary Care
This service provides comprehensive primary care within a concierge framework, with a clear emphasis on lifestyle as the foundation of health.
What This Looks Like:
- A traditional primary care relationship with extended visits and enhanced access
- In-depth assessment of your medical history, lifestyle, risk factors, and goals
- Ongoing care focused on preventing, treating, and when possible reversing chronic disease through lifestyle optimization
- Thoughtful integration of medications when appropriate, but not as the default starting point
Who This Is For
- Want a deeper, more personalized primary care relationship
- Are willing to make guided lifestyle changes to improve their healthspan
- Prefer addressing root causes rather than relying solely on medications
- Value continuity, time, and a long-term partnership with their physician
Who This Is Not For
- Are seeking medication-only solutions without interest in lifestyle change
- Prefer brief, episodic visits typical of high-volume primary care practices
— NON-PRIMARY CARE MODEL
Lifestyle Medicine Consultation
This service is designed for individuals who already have a primary care provider but want specialized, expert guidance in lifestyle medicine.
What This Looks Like:
- A comprehensive initial in-person consultation focused on lifestyle, risk assessment, and health goals
- Development of a personalized lifestyle strategy to address current conditions or future risk
- Ongoing follow-up primarily through telemedicine and secure electronic communication
- Coordination with your existing primary care provider as appropriate
- Access to a broader care team, which may include a health coach to support behavior change and accountability
Important Distinction
In this model:
- I do not serve as your primary care provider
- I do not provide routine primary care services
- The focus is on lifestyle-based consultation, coaching, and strategic guidance
This service is ideal for individuals who want expert lifestyle medicine input without changing their existing primary care relationship.
